Admissions Procedure for Mt. Zion School of Ministry
Keep the following checklist, fill out the application, enclose the $10 application processing fee and submit to the Admissions Department. If you have any questions please call (717) 865-2000 or you may e-mail us at admissions@mzsom.org.
Admission Application Checklist
- Application Form - Complete the application form using a typewriter or print using black or blue ink.
- Send in a recent photo attached appropriately onto the application. (This is for identification purposes only, and will not affect a decision on acceptance.)
- Complete the Medical Questionnaire.
- Have your pastor complete the Pastoral Reference Form. (If pastor is related, reference must be done by church leader/elder/deacon.)
- Have your employer complete the Employer Reference Form. (If unemployed, please send an additional Personal Reference)
- Two personal references must be completed.
- Have your parent/guardian(s) complete the Release Form, (if applicable).
- Write your personal testimony.
- Obtain a letter from your Pastor stating there are no marital issues (Married Students).
- Application processing fee of $10 must be mailed with application.
When you have completed the Application, mail all forms to:
